As a student in Allied Health training coordinated by Boston Reed, you will need to provide various documents in order to successfully complete your training. This page gives you access to the forms related to the required documentation, as well as a few valuable reference documents.
Items identified as RTFs can be opened with any word processors, (Microsoft Word, WordPerfect, Works, etc.). Items identified as PDFs require adobe Acrobat Reader. Download Acrobat Reader now.

Prescreening Assessments (PDF)
- Sample assessments similar to those used the first class meeting to make sure students have adequate math and English Basic Skills.

- Students who require copies of any student records or documents Boston Reed keeps on file will need to fill out this Document Request Form. There may be a charge for document requests.
- Information Change Form (RTF)
- Students should use this form to make Boston Reed aware of any new address, phone number or email.
- Physical Exam Form (PDF)
- A physician or nurse practitioner must complete this form prior to your externship placement.
- Assumption of Risk Form (PDF)
- Students must review and sign this form in order to participate in the Clinical Medical Assistant, Vocational Nursing & Phlebotomy training programs.
